You can join this plan if you are entitled to Medicare Part A & enrolled in Medicare Part B & live in our service area. Some plans have additional enrollment criteria to that are specific to that plan-you can speak to one of our Sales Representatives to clarify what criteria are or use our website’s enrollment tool by clicking here to get these criteria for the plan you are interested in. The Annual Election Period is from November 15 to December 31, each year. Open Enrollment is from January 1 to March 31, each year.

To Enroll in our Plan:

	Click on link below. Please note that upon clicking the link below, you will be leaving our website and continuing on to Medicare's own official site. www.medicare.gov
	Click on “Medicare Health Plans 2009 Plan Data” on the Medicare website.
	Scroll down to “Enroll in Medicare Advantage Plan,” click on “Enroll Online” button on your right.
	Enter your Zip Code and Enter Plan Name: Leon Cares Then click on “Enroll in a Plan.” You will see some of the basic benefits offered by Leon Cares.
	Click on “Enroll Now” button on your right. On this page you will see a complete list of benefits offered by Leon Cares.
	Click on “Continue to Enrollment” button located on the top right of the page and also on the bottom right of the page.
	Click “Continue Enrollment” button on right where you will “START ENROLLMENT.” Provide your personal information by filling out the form. Please have your Medicare card or information ready. Complete the online application, then click “Continue” on the bottom of the page. After you have finished completing the application, click on the “Submit” button.

You have successfully submitted your online application to become a member of Leon Medical Centers Health Plans.
